The school's supplementary form should be completed in addition to the Knowsley Local Authority form (see link below to Knowsley website for in-year applications) by any applicants who wish to provide information to be considered under criterion 3-8 of the school policy and returned directly to the school. This form will be disregarded if the applicant does not also name Halewood Church of England Primary School as a preference on the Local Authority application form.
